Sensory and Communication Barriers in Emergency Settings
Going to a psychiatric emergency department during a crisis is, in itself, a highly traumatic sensory bombardment for a neurodivergent (especially autistic or severely ADHD) individual. The loud sirens of emergency rooms, bright neon corridor lighting, chaos, and crowds push an already overloaded nervous system's "fight or flight" response to its absolute limit.
Additionally, many autistic individuals experiencing an acute crisis may experience temporary speech loss (situational mutism). This communication barrier is often misinterpreted by emergency medical staff as "refusal to cooperate," "catatonic withdrawal," or "oppositional defiance." As a result, when the individual is in desperate need of nervous system regulation, they are frequently subjected to heavy sedatives or physical restraints.

A Neuro-Affirming Emergency Response Protocol
When approaching neurodivergent individuals in emergency departments, clinical staff and family members can apply the following neuro-affirming emergency protocol:
1. Minimize Sensory Load (Low Stimulation): Remove the individual from the general crowded emergency area and guide them to a quieter, dimly lit observation room or corner. Allow them to use noise-canceling headphones or pull up their hood.
2. Open Non-Verbal Communication Channels: Do not force the individual to speak. Allow them to communicate by typing on a phone screen, writing with pen and paper, or using visual cards (pictograms).
3. Avoid Physical Restraints: Unless there is an imminent threat to life, avoid physical restraints. Tying down an autistic individual with severe sensory sensitivities can trigger profound physical panic, hyperventilation, or cardiac distress. Prioritize verbal de-escalation and calm co-regulation.
